The score of a film is the original music you hear while watching it. "Soundtrack" is a more general term that refers to all of the sound you hear (including pre-recorded music and sound effects). While there is technically a difference, the two words are often used interchangeably to mean music. For this card, record your own music and use it in a film of any kind. You can record however you like, even if you just use the voice recorder on a smartphone, or the built-in microphone on your computer.
